5191, the pilot was drunk and tried to take off on the wrong runway, it went boom and 49 ppl died.

5191 crashed just after take-off from Lexington KY on a flight to Atlanta, the exact cause is not known. There is speculation that the plane tried to take off from the wrong runway that was too short for that type of plane.
