You would have to start by getting approval from the NFL. Their website should have contact info that would give you a place to start.

As stated above, the first step is to contact the NFL at 212.450-2000 (unfortunately one can't pull general information from their site) and ask to be connected to the Licensing Information group. They will then gather general contact information and forward to your attention an information pack that outlines the process and general fee structure.
Upon receipt of your formal request they will work with you as they go through their due diligence process, Hope that this helps.
