The upper back has a series of very small muscles and your larger deltoids and latts etc. It is the small muscles that cause the pain. I had the same problem. The trick is not to stress the small miriad of muscles by over exercising the larger ones . That means less movement and less stress. This series of exercises would be much too difficult to describe over the internet and may not be right for your case. Thus, I recommened a good physical therapathy group. But that can be hard to find. I found that the ones with mainly woman therapist to be the best. Alot of the men are ex jocks (like me) who still believe "no pain - no gain" That just don't work unless you are in training. My group's motto was "any pain - no gain". Chiroprators are no good for this. Sorry I couldn't deliver more but by rebuilding the small muscles first soved my problem in six visits and an at home regimine that took no longer than 15 min./day Alternating ice and heat helps. It works the muscles by a little contraction then a little expansion.Of course there are band aids like Ben-Gay and Sports Creme, etc, but those are only pain relievers. Doctors aren't very good for this either. You must get the balance back to the muscle tensions. Meaning pulling force. So don't try to kill yourself with a tough theraphy routine. Heal and balance the small muscles first. Look for a physical theraphy group that understands this philosophy. Interview, call, email ,etc. Key is they usually employ a lot of professional women. Being an ex football player I had my share of theraphys. Later in my adult more sedintary life I found this approach to actually work. I've done quite a few of these theraphy groups. Best of luck it's a lot easier than you think.

You can consult a doctor!

First off, pain is an indication that something is wrong. If you knew what it is, you wouldn't be asking, right? So, let a doctor examine you and take an X-Ray to determine that you do not have arthritis, or a muscle spasm or simply bad posture or maybe you hurt it by doing something awkward or uncomfortable (maybe you moved in a wrong fashion or lifted something uncomfortably) or misaligned vertebrae or an old injury come back to haunt you!

By taking pain killers you are merely disguising the symptoms and not getting to the cause, which may bring about more serious circumstances later on.

It depends on the cause. If you've strained your upper spine, this will help.
Roll up a hand towel so you've got a cylinder about 2 inches diameter. (If it's a small towel, fold it in half first). Now lie on the floor, on your back with the towel running down your spine where it hurts. For the first minute or so, it will hurt - then blissful relief. Don't do it for more than five minutes at a time or you'll have difficulties getting up.

See a physiotherapist from the Yellow Pages, get a massage, manipulation and alignment.
I also have Botox shots in my back, they stop the too-strong muscles from pulling the vertebrae out of line, and pressuring the spinal nerve. Love that Botox, I'm forty, feeling twenty!

I posted a similar question about 4 weeks ago, and i suggest you go to the beach (a beach that has powerful waves) and you go either body boarding or surfing, trust me this really works, cos i was in agony before i went.
Or try acupuncture and stretching.
