butterfly effect and don't say a word don't take place in a mental institution but they are in there. all i can think of at the moment

I remember the movie so well, but absolutely cannot remember the name of it. It's from the 70's starring Burt Reynolds and Dom Delouise - Burt ends up institutionlized after a failed suicide attempt and while there befriends Dom - it's a very funny, yet kinda serious movie.

"One Flew Over the Cuckoo's Nest" is good, even though they absolutely BUTCHERED Ken Kesey's novel.
Recently, I enjoyed "Don Juan de Marco," with Johnny Depp and Marlon Brando.
And, if you're willing to go back a ways, "The Snake Pit" (1948) with Olivia de Havilland is still pretty effective.
