No to both. There are many creatures that can fly and don't have feathers. There are also birds with feathers that can't fly.

I can fly (in a plane) and have feathers (in a pillow). According to popular art, angels have feathered wings and can fly.
In all seriousness though, feathers and wings define a bird. There were feathered dinosaurs who seem to have been able to glide but if it is ever found that they could truly fly, I'm sure they would be reclassified as birds.

Yes, all of birds have feathers and wings but there are some birds that couldn't fly which is called fowls. The examples of fowls is a chicken.

most bird species can fly (not all), other creatures can fly (some insects, some mammals)
all bird species have feathers, I don't know of feathered reptiles or insects.

No. Well first of all, the category birds is too general. We have doves, pigeons, maya birds, etc. And actually, chickens could slightly fly, comparative to humans. Oh, and ducks, swans, etc. can fly too.
