One of the coolest we ever did was a piece of bubble gum. Now don't laugh. Get some old clothes that you and he can part with when the party is over and start applying glue to it in small section. Take candy wrappers, pieces of food boxes, hair ... pretty much any type of trash that isn't really messy, and attach it to the clothes.

How about a time pariod peice. He could go as a Victorian youth, or a colonial person. They allways go over well.
Or if you are into Theater, choose a charactor from his favorite show and dress up as him.
Maybe choose a sterotype he doesn't fit into, such as classic nerd, jock, sports player. preppy, etc...
How about a historic figure, choose a hero or vilian in history and dress up like him. That works well, and is easy to define.
