I know a girl who always answers, "I'm WELL". I don't know if "well" is more appropriate than "fine", but she says it with such gusto that you walk away feeling annoyed.

It was not so much the answer, as it was how long it took to finish the answer. It was as tho the person was giving me their dissertation. Unfortunately, when we ask people how they are, for the most part we are really just being polite. I would say at least 80 percent of the time, we really dont want to know.

I don't recall any unusual responses when I've asked but sometimes when someone asks me I say, "Peachy keen jelly bean". But only if I'm in a good mood.
