Ovarian cysts are common and most women aren't even aware they have them. Cysts (and hemorrhagic cysts) usually develop because the follicle (a fluid filled sac that contains the egg) doesn't open to release the egg.

A hemorrhagic cyst occurs when a small blood vessel in the cyst wall breaks and the blood enters the cyst. This is common and usually painful. If the cyst bursts, blood can enter the abdomen. This is very painful and less common. Most hemorrhagic cysts are self limiting but, sometimes, surgery is required.

I have never heard of a fatality from a hemorrhagic cyst. That is probably because a cyst, that requires surgery, becomes so painful that the woman can't tolerate the pain anymore and seeks medical treatment. Of course, in the past, I imagine there were fatalities from it since medicine was not as advanced as it is today.
