Alright, I have a pretty good shot but it is very inconsistent. What can I do for consistency?

I practice like three hours a day, for those who say I don't get enough practice at it.

It's all about range and form. Take a film of you taking shots at practice and with game like pressure, or better yet, during matches. A lot of things you do well during practice, changes during game time. Elbow sticking out, rushing your shots, did not follow through, lack of balance, that type of things.

Take time to look at your shooting form, and see if there's anything you can fix. In addition to that, game pressure made you take shots at the position that you are not accustomed to. Different distance from the basket, or even different angle than the ones you used to practice, is just enough to cut your shooting percentage 10% lower. Triple threat position and moves will help you getting to your spot.

Have someone experienced watch you shoot from the front and side and give you some feedback. Throwing the elbow out, lack of follow through, inconsistent vertical leap when shootig, drifting to the side all these contribute but you need feedback to understand what your doing.
When you practice, start closer to the basket and work your way out as you show consistency. Dont even shoot a 3 until your ready.

Take a bunch of shots and move your hands differently each time. Once you make one in, then try that motion again about 10 times. Same force, hand movement, and length away from the basket. If that works then you have found your specific shot. Now that you have that set, you can try that shots on different sections of the court. Figure out what works where. Good luck!

Develop a dribble stop move, where you dribble and shot. Then work on your shot, bring your elbow in and follow though each time. Just make sure you take about 25 per reps. Shoot lots of free throws and work on the release.

Probably cause you ain't shooting it right and the ball leaves your hand in a different way each time, that's why it comes off different everytime? Maybe its better if you had someone look at your shooting posture, he/she could confirm that and correct it for you.
No point doing it the wrong way and doing it forever. Won't get you no where no matter how many times you do it.

from personal experience i have tried to picture the distance from the ball to the hoop as a rainbow so when you release the ball, the ball falls into the hoop like a rainbow(this also works on free throws). you can also try to shoot the ball a little higher so it just falls in the basket or you can hit the backboard(the corners of the square or the center)and the ball will bounce in.

make somebody take video of you shooting several times and watch what you do wrong. use a digital camera although not the best picture but itwill show you exactly what you do wrong every time you shoot, maybe you change your hand positioning.
and when shooting the video of you shooting forget the camera is there it is hard not to think about it( i know, i did this myself) but just concentrate and think you are in a game situation.

like all these people r saying. Just practice man. And it doesnt matter what your shot looks like as long as it goes in.

repititons! thats the key to the perfect shot! u just gotta make sure u are practicing your shot right. make sure you shoot the same way everytime. you should see an improvment in very little time.
