Visited one a few years ago in Boston, but I don't remember if it was in the Metro or not. Also - accessible by T?

Yeah - there's a mall in Copley Square, right off the T (green line). There's also the Galleria in Cambridge, a little bit from the Museum stop if I remember correctly (I always took a shuttle there from Kendall Square/MIT). And there are plenty of malls in the suburbs.

Well there are a few depending on where you were and what you were remembering.

The largest is the one at Copley Place/Prudential (nearest T stop @ copley). It's only a couple blocks from there and the Boston Public Library (where the stop is). It's on the 2nd floor (though part of it in Copley Place is multi level) and spans a few buildings, has a movie theatre, lots of upscale stores like Williams Sonoma, Lauren, Swaroski, food court, etc).

There's another indoor mall near Kendall Square in Cambridge. That one's multilevel as well and has normal mall shops including some nice restaurants.

Then there's the markets at Fanuil hall. Though not a shopping mall per se, the north and south buildings have multilevel shopping and include stuff like Crate/Barrel, Disney store, Durgin Park, etc.

Other malls are in the burbs, like Burlington Mall (not near the T) in Burlington, or Chestnut Hill Mall in Newton off of Rt 9).

Most likely though, you're thinking of one of the first malls.

Copley/Prudential is an indoor mall. Other than that all shopping is outside. There are several area malls outside the city, but most are difficult to get to via public transportation. The Cambridge Side Galleria is pretty big, and indoors and if you take the Green Line to Lechmere it's just across the street. But, you're probably thinking of Copley.
