That's one of the very best, unanswered, questions in all of science. It's seems pretty clear that it has something to do with the way the brain works, but we don't really have a clue about that, either.

Nobody knows. That is a question that goes beyond the physical realm, in other words, it is a metaphysical question, it can't be tested in a laboratory. People might suggest they know or say it is linked to physical chemical reactions, but they do not know for sure. How can a physical process such as a chemical neuron firing produce something non-physical like a thought? There is no physical substance to a thought, you cannot prove that you are thinking right now, you just know you are. No one knows for sure, no matter what they tell you.

First of all we know that thoughtd is the act of thinking; the exercise of the mind in any of its higher forms; reflection; cogitation. "Thought can not be superadded to matter, so as in any sense to render it true that matter can become cogitative."
Thoughts are formed when you start planning to do something... you get up in the morning and you want to go shopping for a new microwave. Your thought process kicks in based on something that someone has told you about a particular brand...something about what your grandmother told you... something about what you read, etc.

In humans, it develops from the collective firing of millions of neurons in certain patterns. No one knows precisely which patterns are linked to which thoughts yet, but they're making progress.
