They never made it out. - SIlent hill sits on two different dimensions. the dark dimension and light dimension. When the climax occurred, the mother and daughter were allowed to leave silent hill itself, but not the dark dimension.
Notice how the dad is laying on the couch, and it's sunny out.
The mother and daughter, however, are in a dark, grey house.
For the father, they might seem as ghosts, since they were able to effect the light world (opened the door).

Not really I think that the demon went into the girl Sarah and they are still in the purgatory realm or dimension that it created to punish the people who hurt Alessa. Ther demon came to Alessa because of her hatred, pain and anger and promised her that they would all suffer in her worse nightmares and now that there is noone left in Silent Hill to torment it went into Sarah as she was supposed top be the good half of Alessa. And it leaves the option open for a sequel.
