Record the time the incident occurred and how long it lasted; the names of all those involved as well as witnesses; give a description of the incident (what happened before, during, and after the incident); note any injuries; & note any consequences. Be very decriptive.

Incidence report? As in like for work? Tell your supervisor and he/she should give you the proper paperwork and help you fill it out should you have problems with it.
An Accident report for vehicles should be filled out by the officer on the scene, and by your insurance agent.

an incident report should be factual and to the point. don't use emotional language. consider using point form, listing the facts in order of ocurrence.
be sure to sign and date it.
