it's a cold war classification. The US and their allies are #1, The USSR and their allies are #2. (Depends which one you ask who is #1, but the other is #2.) Therefore, any country not in the US or USSR "sphere or influence" is a third world country.

I think Japan might be earlier, but not the first world country.
