Only happens in the library level and you need to be on legendary. Use stealth when trying to exit the level and sneak up behind the grunt. Walk into it and it will turn around and shoot at you saying that. I did it twice out of 10 times, because it killed me. VERY VERY VERY VERY VERY hard to do.
