What does it mean when someone is charged with breach of peace? And what is the punishment when they appear in court?

the punishment depends entirely on the facts of the case and the criminal history of the defendant.
First offenses will be treated differently than people with several priors or a large number of criminal history points.
The penalty mentioned by the other person who answered so far is the maximum and the maximum is almost never imposed, certainly not for a first time offender.

In Illinois the conduct must be "so unreasonable" that it alarms or disturbs another and causes a breach of the peace. Here it is a class C misdemeanor, punishable by up to 30 days in the county jail and $1500 in fines. Some municipalities have similar ordinance violations which sometimes changes the possible penalties and may affect whether or not you are entitled to a jury trial.

"Disorderly conduct" in some states------e.g., making unreasonable noise and continuing to do so even after having been asked to stop, or engaging in fighting or tumultous conduct, [and there are other forms]. Generally, the crime is a misdemeanor, Class B misdemeanor in the State of Indiana, carries a fine of up to $1000 and a period of incarceration in the local jail for up to 180 days.

This depends on the state law's definition of breach of peace - it there is such a law. Also, courts have some discretion in determining a sentence.
