Mortar (in small quantities like to patch up some brickwork) is mixed by putting it into a big bucket and the adding some of the required water and stirring it together with a trowel or a shovel. Keep slowly adding more water and stirring again until the wet mortar is as thick as a cooked rice pudding. Add enough to eliminate any dry powder places. Do not add too much or you destroy its final strength.
If you want a larger amount (like to make a replacement concrete step) rent a motorized mixer. If you want lots of cubic yards (like a whole patio) buy it ready-mixed and delivered.

For me, I purchase the pre-mixed type, because I have asthma and the powder form that you have to mix causes me to have an Asthma Attack. It should be the consistency of peanut butter (approximately). Follow the instructions on the package -- it will tell you how to measure for the amount of product you need to mix.
