They associate to the tone of your voice, facial expressions and depending on the situation eg. if baby has just taken its first step and you get really excited, smile a lot and tell them how good they are they know they are making you happy and that when you say "Good girl/boy" again in another situation that you are pleased with them again. Usually takes a few repetitions before they get the general gist of a word.

The word good is reinforced by your action when you use the word. If you slapped the kid everytime you said "good" it would take on a whole new meaning.

Children learn from you talking.

If you keep talking to them all through childhood then they learn how to associate words to what they mean.
