maybe it's just habit.....ears are small....and you don't always feel the soap in there.....odds are you'll figure it out sooner or later....it happens...i sometimes leave soap in my hair (now that my hairs buzzed)

Are you pregnant? I am and I forget to put deoderant on one or both arms, shave one leg and not the other, paint one hand and not the other...if not I have never heard of this unless you have horrible memory

I have no idea. Start there first when you soap up and first when you rinse, that should help.
