I'd say about 5 hours to get all the water heated. However, you can have enough for a quick shower in about 30 min. There's a thermostat on it which is sometimes marked in degrees. I imagine your's just says hi, med, lo. Try med or med hi, hi is probably too hot.
Since it's only 3 years old, there may be an owner's manual on line. Write down the make and model and google it. Even if you can't find the exact model, the odds are good that the manufacturer is making one very similar to yours still. Anyway, you just want general directions and informaton, not a part number. So the on line manual for a similar unit should answer your questions.

It depends on how large the heating element (flame based, i understand) is in surface area, and at what temperature IT is burning.
a 2 inch square element at 300 degrees isn't going to heat 40 gallons ANYWHERE near as fast as a 2 FOOT square element at 250 degrees.
Without knowing the make and model of water heater, there's no way for us to give you a correct answer.
I'm sure the owners manual, however, for your water heater has an answer, if you have it.

The temperature is controlled by the thermostat, which you have to set to the desired temperature.
How long it takes is a funtion of the BTU output of the burner, the temperature of the water already in the unit, the ambient air temperature, and the R-value of the insulation in the water heater.

It should take about 10-15 min to heat it up to about 120 degrees.

SINCE I WORK WITH HEATERS EVERY DAY ON THE GENERAL IT TAKES ABOUT 1 HOUR TO FULLY REHEAT THE WATER ONCE YOU ARE OUT.
THERE SHOULD BE SETTINGS ON THE HEATER FOR YOU TO CHOOSE MOST HEATERS WITHIN THE PAST 6 YRS COME WITH A RECOMMENDED SETTING RIGHT ON THE HEATER.

About two hours. If yours isn't heating well, perhaps it needs drained and flushed. The max temp depends on the thermostat setting.

mine takes about 3 hrs......temp control is on the front near the pilot cover, u can set it to max ( 135 deg) o low ( 95 deg)

it depends on how muck water you put in and the temperature og the heat. how hot it gets depends on how long you leave it for .
