Are gay and bi men included in that category or is this a new category where straight men are experimenting? These new categories confuse me.

it's not a new category. Many straight men do have homosexual encounters. Most are teenagers or young men in their twenties still learning and exploring about their own sexuality. And then there are situation such as prison or in the military where there are slim chance of encounter with the opposite sex. Essentially it's just physical sex, a release of one own's sexual tension. Generally there's no emotional attachment to the sex act itself. It's the primary distinction from homosexual.

its simply one male having sex with another male, that's all, now a male who is a MSM may be married with children but becomes a MSM when he decides to have some fun, a gay can be a MSM but a MSM doesn't necessarily have to be a gay, he may be an asexual, or even a man with four wives who on one particular day decides to have sex with a another man.
You can call a man a MSM only during that period of time that he is having sex with another male
