It's different from state to state. Make a list of the items you need changed and then call each of them and ask what their requirements are.

getting married does change your last name! The person that marry's you will send the paperwork in but the minute you say I Do and are pronounced husband and wife your name is changed. You'll need to go to the DMV and have a name change on your drivers license too! And notify Social Security of the name change

You will need an official copy of your marriage certificate. We did it in Vegas so I had to take my marriage license along to because the certificate did not have enough information on it so it depends on what state you got married in. The most trouble I had was with my social security card but it was not a big deal and I got it done the same day.

A marriage license is enough you just notify the people who need to know that you are married and you new name.
