In the US, for the most part, only medical doctors use the Dr. before their names.

Other people who have earned PhDs usually place the degree after their names, and (usually) only when it is necessary, when the display of credentials is required.

In general, it looks pretentious for someone other than a medical doctor to "advertise" their doctoral degree.

An M. Phil. is a Master's degree, usually conferred when a student is in a doctoral program, but has not yet completed the program. Some American universities award the M.Phil. to Ph.D. candidates when they complete their required coursework but before they defend their doctoral dissertations.

(1) Has already been answered. Many people do use Dr. as a title but the Ph.D. emphasizes what type of doctorate the person was awarded. Society also tends to assume that Dr. signifies a medical doctor, not a doctor of philosophy.

(2) According to the UK standards, a Master of Philosophy degree is a two year program. One year of classes and one year of writing a thesis (approximately 40,000 words). The Master of Literature degree is a one year program with a shorter thesis (approximately 10,000-15,000 words).
