Get a ton of fill dirt and get to working on flattening out the grade of that slope. Once you have leveled it out, then you can start raking the dirt and making everything nice and level so that pouring concrete will be a breeze.

The holes in the dirt sound like you have a tile in the ground that has a hole in it. First, I'd see about getting a permit to dig. Then you need to dig down to the tile and repair the section that is broken. How you repair it depends on the kind of tile it is. You will keep getting that hole there unless you repair the broken tile underneath the ground. I don't recommend capping off or blocking the tile, because the tile probably drains an area somewhere that needs drained. Many times these underground tiles can be over 100 years old and ceramic. You may need to replace the whole tile piece or possibly just need to concrete over the hole without stopping the waterflow inside the tile.

To level the sloped backyard you can use stakes, string, and a line level. Fill the yard in with dirt till it is level with the lines on the string. You may need a retaining wall in some areas to help prevent erosion at the edges of the yard.

First find out how deep and for what reason the holes are for.
Then either move dirt from hi to low areas or bring dirt in to low areas (be aware of drainage issues).

Using sand as your base for excellent drainage and pulverised black dirt on the top of the sand. Sand is less expensive and with all soil you will get settling, so plan for that. Too much black dirt is too heavy and will settle more than sand. Black dirt is is more expensive. If you can haul it yourself, you will save a bundle, too!

Before you do anything make sure you don't need a permit then find out if there is a French drain there, if there is you are going to have to work around it or give up the idea.
