a Ph.D. is higher than a master's degree, so there are two ways about it
first you need a BA or a BS degree, that's the degree you get when you graduate a four-year college.
then you can enroll in either a master's or a doctorate program. if you enroll in a master's program, its two years, and you get a masters degree. if you want to continue after that, you enroll in another two year doctorate program, and you graduate from there with a Ph.D.
OR the alternative is to directly enroll in a four-year doctorate program.
starting with a master's program gives you the opportunity to change the area/concentration of your study...if you know exactly what you want to do, then you can go straight to a Ph.D.

Yes. Many Ph.D. programs accept students right out of undergrad. In some of them, you get a Master's along the way, and some you don't. Sometimes a Master's degree is the consolation prize, if you will, for not passing the qualifying exams on the way to the Ph.D. This is particularly true in the sciences.

You have to do the coursework whether they give you the extra degree or not, so enjoy the masters and the really cool sleeves on a masters academic robe. Besides with the masters, you are sure to get something for your efforts. Some Ph.D. students do the coursework and never get their thesis accepted...so no Ph.D.

Yes. In the life sciences (biology, neuroscience, etc.) you can usually bypass the Master's degree. Perhaps other sciences as well.
