Yes. Claudius was Hamlet's uncle. Claudius murdered Hamlet's father who had been king, and so then Claudius became king. Hamlet's mother (Gertie) then married Claudius.
The ghost of Hamlet's father was a-lurking on the battlements one night and told Hamlet he had got to avenge the murder. This is why Hamlet was so upset, what with his mother carrying on with the murderer an'all. Because it was a ghost that had provided him with the information, Hamlet was even more worried about what to do than if an ordinary human had informed him.

Yes, Hamlet says so in the play. Talking about his mother Gertrude, who married Claudius within a month of Hamlet's father dying, he says:

"... why she, even she --
O, God! a beast, that wants discourse of reason,
Would have mourn'd longer--married with my uncle,
My father's brother"
