I just did the same thing.
Be polite and tell her/him you are giving your two weeks notice. Then write up a resignation letter. In both cases, thank them for the experience you had with them, that you wish them future success and state you are leaving due to a change of life. Change of life is a nice way of saying you hate everyone there and would rather be unemployed than show up and deal with the bull****.
Be professional even though the place has driven you mad. You will need them as a reference. Good Luck ! You boss will be pissed, but maybe if they treated you nicer, you would have stayed. (when will employers learn??)

Make sure you have a new job before quitting the old one!!! I cannot stress that firmly enough.
When you do submit your resignation, BE NICE. You've invested a great deal of time with this company and that alone looks really good on a resume. You say, "I've learned a lot here... I want to thank you for the opportunity... I enjoyed working with everyone... I want to seek out new challenges and opportunities." And then ask for a letter of recommendation and don't leave until you get one.
I've burned every bridge I had and really wish I hadn't. Playing politics will get you a good reference.

Just tell your boss that you are leaving for a career change. Be sure to give at least a 2-3 week notice and be sure to show up those last weeks, that way you will at least get a good reference, especially since you worked there so long......

You should either make it a really big scene and tell your boss off, or you should do nice and respectably. Make sure quitting is really what you want. Don't feel guilty for your desicion to quit. You should tell your boss that the job is too stressful for you right now. She/he should understand.

write up a resignation letter, pull him aside and ask to speak with him prvately. lay out your plans and then hand them the letter. this way you can make it face to face and professional as well.
