Their business office or rather headquarters is in the Sears Tower, Chicago. (The tallest building in U.S.) You can get the exact address from the Yellow Pages on your computer.

Good luck. I sent them my credit cards a while ago for the same thing.
I no longer shop there, and that's the most effective way to show your displeasure with their lack of customer service.
