all we carry on our ambulance is Lactated ringers and in the ER they have to replace our bags with normal sailine and flush the line before they start bood infusion. Why does saline work and Lactated Ringers not?

The mixture of the electrolytes and dextrose in the Ringer's alter the plasma proteins a small degree, but it is to the degree that can cause the red blood cells to stack up like coins, called rouleaux formation, and while not a true blood "clot", acts to sludge the blood and make it not flow as it should through tubing and, more importantly, small vessels.

Lactated Ringers has a larger volume than whole blood. IF you infuse it with whole blood than you will be adding more volume to the blood stream causing High blood pressure and fluid overload which can put stess on your heart.

Why would you WANT to infuse blood with Ringer's Lactate?
