If you want to be absolutely sure, AAA (which is one of two organizations recognized by the US State Department to offer international drivers licenses) offers an international drivers license that is recognized in several countries, including Italy. I've been going to Italy every year for at least 20 years (I'm going in October again, I can't wait) and I have never needed one. Whenever I rented a car in Rome, they merely requested a driver's license and a passport. As a general rule, a drivers license and a passport should be sufficent, both for cars and Vespas. Have fun!

You may be coming back in a full body cast. The drivers are NUTS over there. Have you considered a Humvee?
