Many have died during space missions. The first was Vladimir Komorov, whose parachute failed to fully deploy and his craft hit the ground at high speed.

In February 2003 the Columbia orbiter incinerated 15 minutes before it was due to land, with the loss of seven crew. Others have been incinerated on the Apollo launchpad.
Space seems to be a safer place than the earths atmosphere or the launchpad, because no nation has ever admitted losing someone in space.
Contrary to an earlier answer, the Russian, Uri Gagarin, returned to earth safely and became quite a celebrity.

If you stop and think about it my friend, if it wasn't for the fact that we are all upon this little ball in space that we call "Earth". we would all be "lost in space". The Earth is travelling around 67,000 mph through the Orion arm of the Milky Way Galaxy, and the Galaxy itself is travelling at enormous speeds through the known Universe. If we were not upon the Earth, surely we would all be "lost" in the immense blackness of the Universe. But "feeling" lost in space?...yes, many times. It usually happens to me when I feel that my life is stuck or put on hold and feeling so completely alone that I have no one to turn to with the compacity to understand the complexcities of such lost feelings...feelings which, by the way, are very real and so difficult to handle. I have thought considerably about this very thing often-times, and have discovered that "lonliness" is the main contributing factor. The only way through it my friend, is to simply "ride-out-the-storm". Take courage however, because nothing lasts forever...including this feeling of being "lost in space". Things will improve and, remember that old cliche'..."Time heals ALL wounds"...and indeed it certainly does. Good luck...and my prayers are with you.

Not me. I always know where I am.

Some people have died. . Their bodies have not been recovered. I think that counts as getting lost.
