Run cold water directly over the stain at a high pressure (I use the hose from the kitchen sink), and this should help get most of the blood out. Remember--it needs to be cold; hot water will make the stain set.
After you've gotten most of the stain out, use Oxy Clean to help clean the rest of the stain. You may want to repeat both steps a few times before drying the garment, as drying it will only further set the stain.

In all honesty if this is underwear, I usually just throw them away and switch the type of feminine hygiene product I used that caused the leakage. But if its a chair or some pants or something, I would probably use peroxide on it for about an half an hour, then wash using Dreft. Its a detergent for babies. It gets out all those spit-up stains, baby food spaghetti stains, poo-poo accidents, rust, and i'm pretty sure it can get rid of the blood too.
So try this and if Dreft is a bit too expensive ($11.00 for 28 load box), try Purex for babies, it works great too but the scent made me and my daughter sneeze so I prefer Dreft.

Try this, it works:
1.) Make a paste with meat tenderizer and a little water.
2.) Using an old tooth brush, scrub the stain with the paste.
3.) Allow to set a while, possibly over night.
4.) Launder as usual.

Spray and wash might work, or oxy clean... for blood stains, the best thing is hydrogen peroxide. (Test it to make sure it won't damage your fabric.) It breaks up the proteins. Just pour it on before you put water or anything else on it. After it bubbles it all up, rinse with cold water, and put it in the laundry.
I add a scoop of oxy clean or BIZ to my wash usually anyway for the kids' stains. That wouldn't hurt here either.

Use shout gel. Use the brush tip (already on the container) to get it into the fibers...Let it sit for a few minutes then wash the garmet. Also, you can soak it in cold water, but that doesn't always work to get all of the stain out.

if you have not laundered the garment
Grab tons of paper towels and some hydrogen peroxide
fold a paper towel into fourths (in half then half again)
place it under the stain (lay the fabric on the paper towel)
pour a little hydrogen peroxide on and dab it off with another paper towel
remove the paper towel from behind the garment and place a fresh one in its' place
pour on more hydrogen peroxide
keep using fresh paper towels until the stain is gone

oxiclean laundry stain remover spray on the garment and let stand a few minute then put in washer with cold water and use the oxi clean in wash and it will get the stains out.
what I did is use a tub and put cold water in it when you change your underwear put it in the tub and let them soak will be easier to clean if you do that.

Get lye soap. It is a yellow, cheap bar of soap that you can find in a Mexican store or possibly Bashas. It gets out everything. Just rinse the stain with water and then scrub it with the soap like a crayon, then get it a little wet and rub the cloth together.

The dollar store has a product called AWESOME. It just that too. It gets out most stains and I have used it in my washer for white underwear that have been stained from periods.

don't use bleach or hot water - that will make the stain permanent. use cold water and "bio-ad" - bio-ad is a cleaning product designed specifically for getting natural stains (like blood and grass stains) out of fabric. you can't use it on silk, though.
