Buy some salt for freshwater fish, and change half of the tank water with distilled water (extreme purity can be harmful to the leeches' metabolic balance), the leaches cannot survive in it well. Light can also harm the leeches so leave your tank lights on longer. Any medication with copper will also kill them, but it will also de-cycle your tank so leave that option as a last resort.

If you have leeches in with your Freshwater Fish don't panic. Go to the fish store and get a Puffer fish. They love all the junk in your tank. If you have recently put some live plant life in with your Fish that is how they got in the tank. You might see other undesirables creatures in the tank now. For example snails, if you have any other creature besides the leeches, you can use them as food for your Fish. Catch them and smash them up and let your fish feast away. An easy way to make them into food is to smash them alone side of your fish tank, but don't look. Ick... Nothing is wrong with your tanks environment.

best way is to scoop them out with a net every time you see one, you will get a few out every time you try.
