Let's say I'm going to do a two page resume because of really dense combo of experiences and such; if I put my name, address, and phone number on the top of the first page, should I do so on the top of the second page?

In the event the pages get separated you need to have your name on the second page. Here's how. In the upper right corner type the following:

YOUR NAME*Page 2

It is not necessary to include address, phone etcetera on the second page.

As a high school career counselor the rule of thumb I use is to keep the resume to one page unless you have more then 10 years of experience in the occupation you're marketing. And your name, address, and phone number normally only goes on the top of the first page. All too often people put tooooo much information on a resume resulting in the need for a second page. Whatever occupation you are marketing your resume should only target that occupation and ONLY focus on the most current experience you've had usually no more then 5-10 years. And keep in mind that the employer is not interested in reading alot of information. They want to read bullet phrases that capsulate your skills/abilities and work experience.
A one page resume should have enough information for an employer to decide whether they want to bring you in for an interview.
I'd be happy to help you write the resume ...

As a recruiter I say no...your personal information should be on the front page and cover sheet only. I wouldn't worry about losing it in a fax...most resumes are emailed anyway.

Put at least your name and phone number on the second page in case they get seperated.
