It is explosives with nuclear radioactive material packed around it and then exploded scattering the radioactive waste material over a large area, and the area is then uninhabitable.
Depending on the material, how much material, how big the explosion is and also how pure the radioactive material is at the time of the explosion.
The bomb is primitive in nature or design and the materials in the dirty bomb can be radioactive to different degrees, but if one went off in a metropolitan area, it could possibly make the city uninhabitable until it is cleaned, and even worse.

A bomb that has other nasty materials in it besides the main explosive. These can be nuke, biological, chemical, etc.

I think the idea behind the term was that after the explosion, it would leave a toxic (dirty) residue or fallout. So, you get a "bonus" effect beyond the iniial exposive event.
