Those experience days are very nice, and something he can remember for the rest of his life. Something like a track day or driving experience perhaps?
For my dad's 50 birthday I bought him a 2 hours flight lesson which he absolutely loved and still talks about today. It was the happiest I've seen him in years.

Does he like to golf?
Get him golf balls, a gift certificate to a driving range and one to a golf course.
If he is into movies give him movie passes and maybe a movie or a gift certificate to Best Buy or someplace like that.
If he like to have a drink there are Beer of the Month or Wine of the Month or even Coffee of the month that you can order him and they will ship it to him every month or however you want it sent. How about a hot air balloon ride for both of you and whoever else in the family
Good Luck to you

a balloon experience. Flying above your own city, recognising your house with the cold winds in your face, the exciting of feeling in a free-flow and, hey i will not forget the champagne.
complete safe, complete different, perfect surprise to remember.

A pocket watch with in engraving upon it representing your sisterly love is an item that will let him know that when ever he reaches in his pocket to check the time, you are there.
