I don't think all people do. But I can think of some possible reasons. Firstly, most people have skinned their knees at least once in their life, so many people have some scar tissue on their knees. Scar tissue doesn't tan. Also, some people's knees are a little wrinkly. If you lie out, the knee wrinkles and parts of it don't get hit by the sun. Later, when you're walking around, the knee unwrinkles when your knee bends, and the untanned spots are seen. Finally, the kneecap juts out from the surrounding leg when you lie down, casting a shadow on itself and on the leg depending on the sun's position. Those spots get less sun than the surrounding areas.

my knees are very dry and the skin is a bit thicker here, less likely to tan. also i'm always knocking my knees and this removes the tanned skin quicker

The skin on your knees is thicker than it is on any other part of your leg, so it's more difficult to get a tan on it.
