I'm in between sizes: neck 15 1/2 (Medium) and sleeve 32 (Small). I used buy smalls and not button the top button, but now I need to wear ties.

Should I be buying a size up (Medium) and search for a slim fit, and then get them altered? I hear altered shirts have weird darts in the back, but even smalls are pretty baggy. Help.

You can still wear a tie WITHOUT buttoning up the top button. Try it & see if that works or not. Ask a tailor if your shirts can be altered.

There are shirts that can accomodate both of your sizes. Dress shirts are made with a 15 1/2 neck with a 32/33 length on the sleeves. I would suggest that, and if you can't find the size, I would go with the correct neck size with a slightly longer sleeve 33/34 because if you can't button the top button, the tie looks unprofessional. Altered shirts do look slightly off, so wear a proper shirt as if it does fit.

Tailored Shirts will always be best - - - there is simply no uniformity in the human anatomy. That said go for a size larger and make it clear that any darts are to be flattering. Actually a clever tailor knows how to avoid darts.

Or, you can just get some button extenders.
