bottom line, for you this is a lose/lose situation you have walked into,

What are the chances of someone, who looks at herself in a mirror every day, needing to be told what she already knows?

Hi,
If you consider her to be your FRIEND, then you should not think of her that way!
The way she looks like should not judge how good your friendship is!
No one can control the way they look like in the first place “without plastic surgeries” but everyone can control the way they behave and deal with other people.
As for the “bald head” then maybe you can go and search for a good treatment to grow hair back, and before giving it to her just speak to her gently and tell her about it.

Well for one she should know she is bald......and if you are a friend I really dont think you need to tell her she is ugly. Maybe you should help her try to make herself look better by maybe wig shopping or showing her a better way to apply makeup.....use some positive effects they work much better.

I gather your friend is aware of both of these things is she not? Is this medical? A fashion statement? Can he/she afford treatment/correction. If its something unavoidable, get them to go on an outing to the mall or something, tell her you need to look at some new shadows or something, you would enjoy her company and go to the make up counter and play around some. If this was some sort of attention getter of sorts, you as her friend could say, short hair really isnt your thing, wanna go wig shopping?, start her on a collection of colorful bandanas.

I would say it the nicest way cuz you would want to hurt her feelings so here is something I wrote for you to say to her
[You r one of the greatest friends I ever had and I truly love you and I feel like as a friend I need to be honest wit you and to tell you the truth which is so hard for me as it will be for you and I don't want our friendship to end(unless you want it to) But your blad and not verry pretty and you might want to get a weg am so sorry I love you but honesty is the best policy]
