Absolute minimum is 90 kbps in both directions (upstream and downstream); pretty much any broadband connection will support that. If you want to find out how your connection will work with voip, there are a couple of really good online connection testers:
Use voip.com's to find out how many lines your connection will support. (http://www.voip.com/speedtest.aspx) and Brix's to see what kind of audio quality you can expect with the speed you have (http://www.testyourvoip.com/).
The bottom line? The faster your broadband, the better your voip will sound.

You'd have to contact your VOIP provider for bandwidth requirements. dsl or cable connections will usually work.

Depends on your provider, but any broadband connection is good for VOIP.

256kbps is enough for VoIP. But 512kbps makes a difference.
