No, it's not painful. the doc will use an instrument called a speculum to open your vagina a little. It feels a little weird but not painful. If you have a Pap smear, it might be a little uncomfortable when the doc takes the sample, but it's no worse than a baby cramp. Sometimes the doc will palpate your abdomen, you'll feel a little pressure but that's all.
If you do feel pain make sure and tell the doc so that he or she can go a little slower. And try to relax, it's nobody's favorite thing to do but it's not that bad.

First of all, a good ob/gyn or family practitioner will explain what is happening every step of the way. Most people have no pain whatsoever and not even any discomfort. The first time, try to relax and remember it is meant to be pain free. That way you don't tense up and cause yourself pain. They will weigh you, probably take a urine sample, take a medical history and put you in your room. Most doctors instruct you to disrobe and put on a gown with the opening in the front (they also do a breast exam which every woman - and man) needs. Then the doctor will have you put your feet in "stirrups" and scoot to the end of the table. The doctor will exam you. He will look, he will insert usually one finger and, at the same time, push on your tummy to locate the ovaries and the uterus and ascertain the size of them. He will then insert a plastic or metal device that looks like duck bills. This allows him to look at your cervix and to swab it for a Pap test. That's it. You may also have blood work if it is indicated. It's really no big deal honey. Rest easy. The whole time in the office is maybe 15 minutes of being examined and a history being taken.

Well...... in my experiences with pelvic exams is that they can be uncomfortable because the instruments used during examination can be colder than your body temp. Nothing about it is painful. The doctor will press certain areas of your stomach during the exam and that sometimes can be uncomfortable. The whole process takes less than 10 minutes and off you go to enjoy the rest of the day.

Generally, it's not painful (unless there is a problem...)
You didn't say why you're getting a pelvic exam, but your doctor can help you to know for sure if you need one. Indeed, I recommend you meet with the doctor before the day of the exam to make sure you are comfortable with him/her and you can get all your questions answered.
As far as what they do, try checking out

If you don't mind some advice about this from a guy... call the office where you will be getting your exam and tell them you've never had one and ask what it'll be like. They do this all the time and will understand and help you feel more comfortable about it. I've never had one of course, but I've had to have other, uh, procedures, and they helped a lot when I called ahead.

It's not painful. I picture myself on the beach and before I know it, it's over. Doesn't take too long....it's uncomfortable but very necessary. Good luck.
