Which is grammatically correct? I see both so often that falling back on "what looks right" isn't working for me this time.

BOTH ARE WRONG!

a.m. and p.m. MUST BE abbreviated! they stand for something!

a.m. = ante meridian (before the sun...)
p.m. = post meridian (after the sun..)

So AM is worng, am is wrong, the correct is EITHER A.M. or a.m. - i would use a.m. in correspondence

The correct grammatical use is "a.m." and "p.m."
this is the correct format used in business correspondence.
The periods are always included in the correct use, but it really does not matter unless you are corresponding with a business, applying for a job, or turning in a school paper.

As far as I know the lower case initials are correct. Ante meridian and post meridian are not proper nouns and so are not written with capital letters, therefore their abbreviations would also use lower case letters.

Capital AM for anti-meridien or PM for post meridien. Grammatically, initials are always capitalized.

I think it can be either. But most of the time I use "am". I think "am" is preferred though, but I am not exactly sure.

I usually put a.m. or p.m.
With the dots, and lowercase.

am - in a sentence as a verb
