Actually even though he's no longer in office he is still called President Clinton. First lady is not an official title but I do believe many would just refer to him as the first gentleman or first man.
Some how I'm scared and amused by this question

If Hillary Clinton were President, we would call Bill collect from another country if his wife was running the U.S.

The former President, Bill Clinton or first gentleman. If you knew him you would probably call him Bill.

Probably " the First Gentleman". However, it would also be correct to address him as "President Clinton" (which honor he has for life) but not "Mr. President", which is reserved for the current holder of the office.

First impeached president. Whoops, he was not the first, how about Mr. Impeached President. Before I start getting bashed, check your facts. He was impeached, he was not disciplined which is why he remained in office. More the shame.

Makes me nauseous to even think of that possibility! But IF it happens, wouldn't dignify him with a title that could be used in mixed company...just "plain ol Bill".
