Yep, it's possible. The exact technique varies from doctor to doctor. Basically, the male genitalia is removed and some peices of it are used to create the lips, vagina, etc. If the doctor is competent, there shouldn't be any difference appearance-wise between the vagina of someone born physically a woman and someone that undergoes a "sex change." People that have that done urinate the same way any other woman does.
*People that have this done are transsexuals.* When born their mind is female, but their body is male. They just undergo some physical changes so that they're both physically and mentally the same. *People go both ways,* but many of the people that change their bodies to be physically male don't have the male parts added, as the results typically aren't nearly as good.
Also, many of the tissues are essentially the same stuff. For example, the clit and the penis are made of pretty much the same tissue and both contain a lot of nerves. For people that are physically female but mentally male that switch sides (FTMs) and take testosterone, the clit enlarges to the sive of a small penis. For those that are attracted to women, it's frequently large enough for them to penetrate their female lover.

This is entirely possible and with the new techniques the results are excellent. The candidate must go through intense testing both physical and psychological, take hormones to develop breast and reduce male characteristics and be approved for the final step which removes the penis and using part of its skin a vagina is developed.
