I used to translate for IEP/Special Ed. teachers in California, and the general format was a collaborative effort. The teacher of Special Education and all of the other teachers were involved, the PT, the OC, the Speech Therapist were sometimes in the same room at the same time with the Special Ed teacher while they worked with their children. They also made assessments while the Special Ed teacher was present (the Special Ed teacher also made her own assessments.) Usually they shared information before the IEP meeting with the parents.
