It comes from the origin of the word "testify". During the days of the Roman Empire the soldiers swore an oath, during which time they would put their fingers on one of their testicles. They swore allegiance to the Empire, etc., or their commander could order the severance of their pay... oops, I meant testicle. To "have balls" is to be willing to go against the Empire, which would take some real courage.

Most self proclaimed "he men" think the world is just a big pecker contest. Also, most of these types have little tiny winkies, so they drive big "ram" pickups with dual rear tires, with a big diesel engine, and never ever have a load in the truck. They watch someone else play football so they can feel macho, and memorize all the stats because it makes their pee-pees feel bigger (they can't actually see their pee-pees because of their macho beer bellies). When you think about it, it's actually the wimpy little four-eyed nerds who rule the world. Skinny people actually have bigger winkies, because they're not half buried in fat. Courage is NOT lack of fear (that's just plain stupidity). Courage is being scared shitless, but doing what's needed in spite of the fear.
