About 4 years ago I changed my name legally in FL (first, middle, last) and it was like $80. Some of my family members would use my name and SS# so I thought changing my name would get them caught in the act...they found out the new name...
Call your local clerk of courts and they will tell you.
All I had to do was go downtown and get the forms (which you can usually get online these days), fill them and turn them in with my money and a self addressed stamped envelope for them to send me the paperwork back in. I didn't have to go to court but I did have to wait for the judge to sign the papers (took them damn near two months) and then when I got the paper in the mail I had to take it to the court house to have it stamped. You will need to keep that to show all the people you need to change your name with (credit card companies, landlord, etc) and it is a big pain in the butt. You need a new SS card (same #), new birth certificate, new ID, etc.

It depends on where you live, what the court filing fee will be for your area, and whether or not you decide to hire an attorney to help you.

It depends on where you're at. It seems to hover around a hundred dollars, though. Expect to go to court, too.
