I find the best way is to put the jockstrap on first without the cup, the Bike tag should be facing outward you can then slip the cup into the pouch making sure everything is covered by the cup. You don't need to wear any conventional underwear over the jock if you don't want to, its really personal preference. I prefer not to as I find you get too hot and run the risk of 'jock itch' a sort of fungal infection similar to athletes foot, believe me its not nice! I would also ignore any mad suggestions that you wear the jock over briefs or boxers the whole purpose of the jock is to support you wearing it over conventional underwear defeats the object.

I would put the jockstrap on first, label side away from the skin, then insert the cup. It's easier to adjust the jockstrap for comfort before putting the cup in and you can get better comfort from the placement of the cup. I don't see any need to wear anything on top of the jockstrap.

Its generally easier to put the cup in prior to slipping the jock on. The tag should be on the outside. The only reason you might wear boxers over is to "hide" the evidence of the strap lines when you bend over. (If you have light colored pants) I always showed off personally.
