No, I attended a religious school in NC, High Point University. They ask you on your application your denomination but that is it. However, be prepared to take one or two religion based classes that will be required to graduate.
Going to a school that is religious may not be for you if you don't share the same religion. You will find most of the functions and programs on campus focused on religion and religious beliefs. They will most likely have a church right there on campus. And I speak from experience it will be noticed if you do not attend.

No, you don't have to be of that affiliation. I went to a school that was affiliated with the United Methodist Church (Brevard College - http://www.brevard.edu/) and even though I am a United Methodist, there were a lot of people there that were not United Methodist.
I can say that Brevard College does require one Religion class. They aren't taught like Sunday school classes but more like a History class.

You might have to check with the individual college. I would guess that no you do not have to be that religion in many cases. However, as a student you may be required to abide by any rules of conduct they may have set down, possibly including attending religious functions or services on a regular basis.

Usually you don't have to be. You might have to take some classes about religion, but those don't always focus on the particular religion of the school. For example, Jesuit university might offer a class in Buddhism or another religion.
