In the open air (nothing), a fully enclosed holder or one that just encloses the bristles/head only? Wouldn't this be a haven for germs to just grow all over?

In your bathroom away from the shower, toliet and sink. You can enclose the whole brush or just the bristles but like you said this can breed germs. Because of the moist,warm air in your bathroom enclosing your toothbrush may cause the breeding of germs. In the open the brush is still going to have the possibities of getting germs growing on it but not as high as the other choices. Before you brush let the hot water run until it is really hot and then let the water run over the head of your brush for 30 seconds, this will kill out most of the germs on the head right before you brush.

Put a small amount of bleach in a cup, add water, and soak your brush in this when not in your pie hole. You should change this every few days.

Just a few drops of bleach is all you need with 8 oz of water.

Put your toothbrush in a locked, time-release vault thirty miles below sea-level. Set it up so it only opens once every four years.

unless you're severely immunocomprimised, you're not going to get sick from your toothbrush, i can pretty much 100% guarantee that

anything growing on your toothbrush is growing on your skin and all over your bathroom anyway

This does not directly answer your question, but many dentists recommend that you put your toothbrush in the dishwasher when you run it.

doesn't matter. you should be brushing and spitting. If you are swallowing the toothpaste then you should be worried.
